• To access the online forms, select “Proceed to Online Forms” below.

• You will then be prompted to create a Parent Account or Continue as Guest to complete the online forms.

• Click HERE for instructions on how to create a Parent Account.

• In order to complete the forms, you may be asked to provide your student’s ID number and/or Date of Birth along with other demographic information. Please note that you will have to provide an email address at the bottom before being able to continue to next form.

• When uploading a picture of physical and insurance card (front and back) please make sure the picture is clear and has all information in the frame or it will get denied.

• All forms must be completed before you can participate in athletics at Howard College.

• Insurance: Howard College is requiring all student athletes to provide proof of a primary medical insurance plan for themselves to be fully protected while participating in any Intercollegiate Activity at Howard College. It would be the responsibility of the student athlete to maintain this coverage/insurance throughout their status as a participant in a program.
